I had to go up for the shoulder / chest width, and to be able to throw on a sweater, etc under it. The waist is still too big on it for my preference, but that's what the belt (...and potentially a good leatherworker/tailor...) is for....
Yes - that was my exact experience as well. I could have pulled off a 44 in the 613SH (...which is still one size up from "normal" in other garments...) but had to go to the 46 in the 618HH.....

Well, just like Occam's Razor, the simplest answer is usually the correct one - because there are winter days here in the Northeast, where I may need to wear it under the jacket !
