5'9", 190. I have a Cossack in size 42. It is drawn in all the way, and I'm sure I could gain some girth and still be fine. They added inches to the length 'cause I wanted a longer fit. I'm saying you cant' go wrong either way. Tell them what you want and you'll get it;-)
Wow. If that leather is of any substance at all it would have to weigh 15-20 lbs. If it absorbed water in a shower it'd gain a lot of weight! My experience with buffalo is that it is fairly open grained and consequently absorbent.
I have the Cossack, which as you likely know, is a Stuart with a zipper and no breast pocket.
The button sleeves are a little snug, but I like them. With the weight and body of the leather, I wonder if the storm cuff sleeves don't 'swallow' your hands as you move? The button sleeves...

I must agree with Inusuit. They are a somewhat utilitarian, but if you really wear it a Carhartt made of brown duck is the way to go.
They are affordable and serviceable. Brush, barb wire, snow and birdshot will all be handled by the brown duck!
