I've seen some hat size labels that simply say 7/8 but this could be 6 & 7/8 or 7 & 7/8. Which is it implying?

Almost always 6 7/8. In the decades before ours, 7 7/8 was almost unheard of. When they are 7 7/8 the 7 is usually included. But, always ask for a quick measurment for you never know....
